I had this problem too. I had to take the whole thing apart and strategically apply hockey tape to the blade holder. It was pain to get it back in, but now the thing is super solid.

Are you sliding it out in the right direction? I tried pushing out through the pommel side at first, but that wasn't working, so then I just pulled the blade out from the emitter side, and that did...

It's a 2005 ROTS Anakin. I took it apart again and a wire had indeed come loose. I put it back together, and it still does the weird glitchy on-off thing, but way less frequently now.
