The picture of this (http://www.thecustomsabershop.com/MHS-speaker-and-battery-holder-P315.aspx) item shows 3 wires coming off of it. However the battery pack normally has 2, and a speaker normally has 2. So I would think there should be 4 wires. Am I missing something? Could someone please explain how this wires up.

There are 4 wires, it's just the angle of the picture.
Red and black are battery
Blue and Green are speaker

BTW, blue is the speaker positive and green the speaker negative.

Although in GENERAL (with miniature high power speakers that are functioning in a "mono" world)... speaker + and - are irrelevant.
It's almost the same idea as wiring up a vibration motor. Polarity becomes quite irrelevant.
